Description

The Case Management Specialist will be an energetic “people-person”, and an enthusiastic team player who is comfortable working in the field as well as in an office setting. The Case Management Specialist must have a previously demonstrated ability to effectively interact with individuals and families that or homeless or at-risk of becoming homeless. The Case Management Specialist is responsible for facilitating any resources that may be available in order to meet the needs of the families. The position requires effective coordination with other organizations as well as a significant amount of written documentation in the form of notes and data entry into the Homeless Management Information System (HMIS). The position requires written proficiency and the proven ability to work with groups. Under the supervision of the Case Management Supervisor, the Case Management Specialist will be expected to carry out the responsibilities with only a limited amount of supervision.

Principle Duties
  1. Completes assessments to determine family/individual strengths and barriers, including employment, behavioral health, criminal history and substance use to determine appropriate levels of housing and rental assistance
  2. Meets with assigned families at home or in the office on a weekly basis. Assist families and individuals with establishing and meeting housing goals and identifying appropriate steps towards achieving goals, including modifying goals as needed and evaluation of progress
  3. Arrange for and/or assist in providing basic training for families in the areas of household maintenance, financial literacy, safety, rules of conduct, and the governing policies and procedures for Samish Indian Nation’s housing as they may apply to Program Participants
  4. Identify any special needs of assigned Program Participants, and coordinate with a variety of outside service providers, including coordinated entry sites, food banks, dispute resolution centers and other service providers as appropriate to facilitate an effective relationship between the special service provider and the family in need
  5. Landlord Tenant engagement – identify housing options and negotiate tenancy for program participants with landlords and property managers that will result in Participants becoming self-sufficient
  6. Conduct annual, move-in and move-out housing inspections and meet with the Program Participant to review findings and establish a plan for repairs, if applicable
  7. Prepares Monthly reports for the Housing Director- Rental Assistance Codes, Master Rental Updates, Homeless Management Information System (HMIS) data entry, Recertification and Housing Inspections
  8. Attends local, regional and national meetings and training workshops related to homeless and prevention programs and services
  9. Maintains a clean and professional work environment, adheres to all personnel and financial policies and procedures, and works cooperatively with Housing Committee, Samish Tribal Council and employees
  10. Perform other duties as assigned

Required Qualifications
  1. Bachelor’s degree in counseling, psychology or related human services filed or the equivalent years of experience plus a minimum of two years working with homeless or at-risk of becoming homeless individuals and families
  2. Must become a HUD Certified Visual Assessor within 30 days of employment. This is accomplished by successfully completing a 20-minute online training on HUD’s website
  3. Experience with or ability to learn the Homeless Management Information System (HMIS) data reporting system
  4. Significant proficiency in Word and Excel
  5. Must have the ability to communicate effectively, both orally and in writing, with funding agencies, attorney, contractors, Tribal Council and staff, and the public
  6. Responsible for upholding a culture of privacy and security in highly confidential work environment
  7. Complies with all tribal policies and procedures that involve access to and safeguarding of client protected health information
  8. Demonstrates cultural competency, respect for diversity, and possesses ability to work independently as well as part of a team
  9. Must have a valid Washington State driver’s license and be able to meet the minimum insurance requirements regarding driving record in order to utilize GSA vehicles

Working Conditions

Physical Demands: Long periods of sitting, including driving (60% of time); occasional bending, crouching, stooping, stretching, reaching, lifting, carrying, and pushing (20% of time); and occasional standing and walking (20% of time). Occasionally lifts/carries up to 20lbs. Must possess manual dexterity sufficient to use office equipment with frequent repetitive motion such as writing, typing, paperwork handling, and phone use. Must possess the ability to view print on a computer screen and/or paper document; the ability to speak and hear exchanging information with end users by telephone. Mental/Visual Demands: Frequent mental and/or visual attention to task is required. Occasionally work requires heavy concentration. Frequent interruptions. Tasks involve eye-hand coordination. Working Environment: Work environment is mainly indoors with heat and some air conditioning. Must sit or stand at desk for the majority of shift. Occasionally works alone without benefit of supervisor or coworker. Unavoidable hazards/risks: Frequent vehicular travel; work requires minimal risks which require safety precautions (using some cleaning chemicals for light cleaning). Protective clothing or equipment may be required (goggles, gloves, masks, boots, etc.)

CONFIDENTIALITY: This position develops, maintains, and is responsible for safeguarding the computer/electronic and hardcopy file folders containing sensitive and proprietary information about the Tribe and its members. The incumbent must maintain confidentiality of all work and files pertaining to enrollment, clients, survey results, and all other material that may identify individuals by name (either directly or by reference), as set forth by the Tribal Council in the Personnel Manual. Any violation in this matter may result in immediate termination of employment.
