Updating Your Information
If you relocate from outside our Purchased and Referred Care service area into one within our ten-county service area (Whatcom, Skagit, San Juan, Island, Snohomish, King, Pierce, Kitsap, Jefferson, or Clallam counties), proof of your new mailing address such as a utility bill will be required. For any other moves, please call or email with your new address. If your phone or email changes, please reach out via call or email. In the event of a tribal citizen's passing, an obituary or a copy of a death certificate is required.
